If a student is only 15 and applies for attunement, I would request parent/s signature and concent.
If a patient discloses something of a criminal nature, that is harmful to him/herself and/or others, I feel responsible to deal with that within legal boundaries, and have included this in my disclaimer form. In such a case, the patient would be encouraged to take measure to inform appropriate authorities, so that they are responsible for themselves. If they don't, then it would be my duty to take action for the sake of safety of others.
Do other Reiki Masters have similar approach?

Sounds like a good policy. I've only used the standard disclosure which states something to the effect that reiki is for relaxation only and is not a form of medicine and is not massage therapy.
